VENDOR INFORMATION - Note: Vendors within the City
Limits must be locally sponsored. Food vendors must get permits from the
Linn County Health
Department Turn in to Gwen's by 6/3/08.

Vendor application and submit
it with the $100 fee. Make check out to Lisbon RAGBRAI
Committee and submit to Gwen at Gwen's Restaurant no later
than June 3, 2008. Forms are available Beryl at the Lisbon
History Center. Proof of insurance forms are due by June
3, 2008, to Gwen.
Sales Tax permits must be turned in to Gwen by July 1, 2008
(except 501(3)c organizations using the money within their
organizations). Sales tax permits may be obtained
online from the DOR at
www.state.ia.us/tax.
The County Health Inspector, Heidi Peck, may be reached at
heidi.peck@linncounty.org to answer questions regarding
food, health standards. etc.
